As a Master Guide, Pathfinder or Adventurer Club Director under the Minnesota Conference of Seventh-day Adventists, I acknowledge and accept my responsibility to ensure that all required documentation for each club member is properly collected, maintained, and submitted. This includes, but is not limited to, individual Health Forms and Emergency/Consent to Treat Release, Code of Conduct Policy and Photo Release Waivers, in compliance with the established Minnesota Conference protocols and the regulations of Adventist Risk Management for Youth Ministries. I understand that these forms are mandatory for the safety and well-being of all participants, and that failure to collect and safeguard this information places the club, the conference, and its members at risk. By signing this agreement, I commit to diligently fulfilling this responsibility and upholding the standards of safety and accountability required of my role as Club Director.
