2021 VIRTUAL CAMP MEETING
PARTICIPATION AGREEMENT AND COMPLETE WAIVER AND RELEASE FORM

I have chosen to allow myself/my child/my family members to attend the Minnesota Conference Virtual Camp Meeting programs which includes all activities.

1. Risks. I understand and acknowledge that participation in the virtual meetings offered by Minnesota Conference entails risks both known and unknown, regardless of negligence which could result in serious physical or emotional injury, disability, paralysis, and/or death, or damage to the participant, to property, or to third parties (“Risks”)

2. Release. I understand and acknowledge that such Risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. I further understand and acknowledge that the Minnesota Conference has no control over factors that may influence these Risks, including but limited to equipment manufacturer's malfunction and a participant's fitness and abilities. I am knowledgeable in the activities involved in Virtual Camp Meeting and have no medical reason why participation is not advised.

I hereby voluntarily and knowingly accept full responsibility for encountering all Risks known and unknown and expressly release and discharge, indemnify and hold harmless to the fullest extent permitted by law, Minnesota Conference, its officers, directors, employees, agents, volunteers and assigns from any and all liability, claims, demands, costs and expenses, and causes of action whatsoever arising out of or in any way connected with any property loss and/or any bodily injury including death and/or disability arising from myself/my child/my family members' participation in Minnesota Conference Virtual Camp Meeting activities.

4. Photo Release. I am aware that photos/videos may be taken of myself/my child/my family members at Virtual Camp Meeting. I grant Minnesota Conference full permission to use images, recordings or any other record of myself/my child/my family member while participating at the Virtual Camp Meeting for Minnesota Conference’s own purposes, which include but is not limited to promotional purposes.

BY ATTENDING AND/OR PARTICIPATING IN THE ACTIVITY, YOU ARE DEEMED TO HAVE GIVEN A FULL RELEASE OF LIABILITY TO THE RELEASED PARTIES TO THE FULLEST EXTENT PERMITTED BY LAW.

7. General. I have read this release of liability and assumption of risk agreement fully, understand its terms, understand that I have given up substantial rights by signing it and sign it freely and voluntarily without any inducement. It is also my responsibility to report any personal physical conditions that could impact participation by myself or family members, and, also report any unsafe conditions that I may encounter to a responsible party.

If for any reason, I do not consent to any or all of this waiver, I will not sign this form and I will contact Minnesota Conference in writing to see if accommodations can be made.
